Making a Difference Where it Counts

This program helps to better serve our community by improving access to care and preparing providers to meet the needs of diverse patient populations.

Participating in a fellowship builds confidence, reduces burnout, and improves outcome. Providers will feel more confident and prepared, which leads to better patient care, stronger care teams, and improved access—especially in primary care and underserved communities.

Cone Health’s Value-Based Care Model

Our care model delivers whole-person care, meets patients where they are, and aligns success with healthier communities.
