Appointment Guidelines

• Before your scheduled appointment with us, you will receive two reminder phone calls.
• Arrive 15 minutes early for your appointment. Bring your insurance card and a list of your current medications.
• Most patients will be asked to provide a urine sample at check in.
• If you need to cancel or reschedule your appointment, please do so within 24 hours of your appointment

After-Hours Calls

To receive care or information after hours and on weekends, call our direct line -- (336) 227-2761  -- to be redirected to a provider on call.

Payment Information

• In most cases, you are expected to pay in full or pay your full co-payment for all office services at the time of your appointment.
• If you cannot provide proof of insurance, you will be totally responsible for payment of the bill at the time of your visit.
